Claim your Welcome Cheque by completing the online claim form at http://btoffer.onlinerebates.com before 30th November 2008. If your submission is eligible for the welcome cheque, it will be sent out within 28 days from receipt of submission. If you do not complete the online form correctly, you will not receive your cheque.
